United Federation of Martial Arts (UFMA) - Waiver and Release of Liability
I, the undersigned tournament competitor and/or Parent or Legal Guardian of the competitor, hereby acknowledge and fully understand that participation in the United Federation of Martial Arts (UFMA) World Championship involves inherent risks, including but not limited to the risk of severe injury. In consideration of being permitted to participate in this event, I, for myself, my heirs, executors, administrators, and assigns, hereby waive, release, discharge, and hold harmless UFMA, its officers, directors, employees, agents, successors, volunteers, and representatives from any and all claims, demands, liabilities, losses, damages, injuries, or expenses, whether caused by negligence or otherwise, arising out of or in connection with my (or my child’s) participation in the tournament, competition, or any related activities. I further acknowledge that I am voluntarily assuming all risks associated with such participation and accept full responsibility for any injury, loss, or damage that may result. By signing below, I confirm that I have read and understood this waiver and release of liability and agree to be bound by its terms.
