By signing this application, I certify that all information provided is true and accurate to the best of my knowledge. I understand and agree that UFMA reserves the right to revoke my membership and/or affiliation, without prior notice, if any information provided is found to be false, inaccurate, or misleading. I agree to pay the applicable affiliation fee for one year of membership upon receipt of the payment link.
Please send to info@ufmartialarts.com: 1- A complete roster of all dojo members (use dojo member form found on website). 2- A list of all organization officials including rand and residence address. 3- Copy of all your martial arts diplomas and any relevant certificates including but not limited to Certificate of last Dan obtained, awards, etc. 4- Copy of Bio/CV/Resume All member schools will be subject to inspection by the UFMA Regional Director and/or members of the Board of Directors. It is specifically understood that your organization shall make every effort to obtain 100% of your total members for membership in the United Federation of Martial Arts.
